Job description

Location: Covering bases across Derbyshire, including Derby, Chesterfield, and Ilkeston, with 9 other peripheral bases operating at weekends.

Working Patterns: Bank Contract, 12-month Fixed-Term Contract, covering evenings, overnights, and weekends (minimum requirement of 2 shifts per month).

Competitive salary: DHU Band 8b.0, £62,484 per annum (pro rata) with a 30% evening enhancement and 45% weekend enhancement.

Job responsibilities

What your typical day looks like:

  • Providing independent, advanced clinical care for patients aged 12 weeks and older, including assessment, diagnosis, treatment, and referrals.
  • Performing and interpreting diagnostic investigations, identifying when specialist input is needed.
  • Engaging clearly and confidently with patients and carers to support informed decision-making.
  • Collaborating within a multi-professional team while maintaining accurate clinical records in line with professional standards.
  • Prescribing medications within non-medical prescribing guidelines and ensuring safe discharge or appropriate referrals.

To be a successful Advanced Practitioner you'll need:

  • Registered and practicing with a recognized professional body.
  • Registered as a non-medical prescriber.
  • Completed at least two Level 7 modules, including Minor Illness & Minor Injury.
  • Ability to work autonomously within multi-professional teams.
  • Skilled in assessing and managing complex conditions in both adults and children.
Person Specification
Experience
  • Substantial autonomous practice working within a multi-professional team.
  • Evidence of autonomous physical examination and assessment skills in practice.
  • Broad and sound knowledge of complex and common conditions in primary care in both adults and children.
  • Experience in one of the following areas: nurse-led clinics, emergency care centres, out-of-hours facilities, or walk-in centres.
  • Consolidated independent prescribing experience.
  • Experience in telephone triage and emergency care.

  • Ability to complete concise clinical histories and physical examinations.
  • Formulate differential diagnoses and management plans.
  • Willing to participate in service development.
  • Awareness of accountability in clinical practice.
  • Effective communication skills, both verbal and written.
  • Negotiation skills and critical thinking.
  • Ability to identify individual client needs and prioritize effectively.
  • Commitment to lifelong learning and continuing professional development.
  • Excellent interpersonal skills and flexibility to meet service needs, including working unsocial hours and traveling between sites.
  • Full driving license is essential (for LLR Home Visiting Service).
Qualifications
  • Registered Adult Nurse (NMC), Paramedic (HCPC), or Pharmacist (GPhC).
  • Registered as an independent non-medical prescriber.
  • Educated to Masters level or equivalent, with an MSc in Advanced Practice.

Salary: £62,484 to £72,865 per year, with a 30% evening and 45% weekend enhancement.
