Job summary

Rolle Medical Partnership in East Devon seeks an experienced Advanced Nurse Practitioner to deliver autonomous, high-quality clinical assessment, diagnosis, and management across a wide range of conditions within a supportive multidisciplinary team.

You will help develop services, contribute to clinical improvement, and participate in education and development of colleagues while upholding patient safety and evidence-based care.

Qualifications

• Advanced clinical assessment and examination skills.
• Ability to take a comprehensive clinical history.
• Ability to assess, diagnose and manage a broad range of acute and chronic conditions.
• Ability to formulate differential diagnoses and management plans.
• Ability to prescribe safely and effectively within scope.
• Ability to request and interpret investigations appropriately.
• Ability to recognise red flags and deteriorations.
• Ability to make appropriate referrals to primary, community and secondary care.
• Work autonomously while recognising limits and when senior advice is required.
• Strong evidence-based practice and clinical governance understanding.
• Excellent clinical record keeping.
• Knowledge of safeguarding adults and children.
• Understanding of infection prevention and control.
• Commitment to patient safety and high-quality, person-centred care.

Responsibilities

• Undertake comprehensive clinical assessment of patients.
• Assess, diagnose and manage a wide range of acute and chronic presentations.
• Provide autonomous clinical decision-making within your scope of practice.
• Develop and implement treatment and management plans.
• Prescribe safely and appropriately within your professional scope.
• Refer patients appropriately to secondary care and other services.
• Recognise and manage deteriorating or acutely unwell patients.
• Provide safety-netting and appropriate follow-up.
• Work collaboratively with GPs and the wider multidisciplinary team.
• Support continuity and high-quality patient-centred care.
• Contribute to clinical audits and quality improvement.
• Participate in MDT meetings and clinical governance activities.
• Support and contribute to education and development of colleagues.
• Maintain accurate, contemporaneous clinical records.
• Work within practice policies and governance frameworks.
