Job summary

HCRG Care Group is seeking an Advanced Nurse Practitioner/Advanced Clinical Practitioner to lead online patient request reviews and clinical decision-making within The Loxford Practice network. You will work with GPs and allied clinicians to deliver safe, effective, and timely care through digital triage and face-to-face assessments as needed.

As an autonomous clinician, you will manage a range of presentations, escalate when necessary, and contribute to improving patient access and outcomes.

Qualifications

• Current professional registration with the regulatory body.
• Advanced clinical practice qualifications and experience.
• Strong clinical assessment and risk-management skills.
• Experience managing diverse patient presentations and escalation when needed.
• Excellent communication and organisational skills.

Responsibilities

• Review, assess and prioritise online patient requests.
• Use advanced clinical reasoning to determine care pathways.
• Manage presentations via online platform within scope of practice.
• Identify red flags and escalate urgent cases when needed.
• Coordinate with GPs and allied clinicians to deliver coordinated care.
• Maintain clear clinical documentation and governance compliance.
• Contribute to improving triage pathways and patient experience.
• Pursue ongoing professional development and supervision.
