Job summary

Rotherham Doncaster & South Humber NHS Foundation Trust is seeking an Advanced Nurse Practitioner/Advanced Clinical Practitioner to perform clinical triage and assessment, undertake autonomous telephone assessment, and determine the appropriate care pathway for acute, urgent and same-day problems. You will work with the practice administrator and multidisciplinary team to ensure timely clinical responses.

Responsibilities

• Clinical triage and assessment of acute, urgent, and same-day problems.
• Autonomous telephone assessment and triage.
• Determine appropriate clinical pathway: telephone, face-to-face, GP review, or referrals.
• Provide treatment, advice and safety-netting; arrange investigations and follow-up.
• Work with practice administrator to manage daily clinical load.
• Collaborate with GPs, nurses, pharmacists and the multidisciplinary team.
• Uphold clinical governance and information governance standards.
