Church Lane Medical Centre in Scunthorpe is seeking an experienced Advanced Nurse Practitioner (ANP) / Advanced Clinical Practitioner (ACP) to join our clinical team. The role is 8am to 2pm four days per week onsite, focusing on telephone-based triage and same-day assessments.

You will work autonomously using advanced clinical assessment and prescribing skills to assess undifferentiated presentations, determine risk, provide treatment and guide patients through appropriate pathways.

Qualifikationen

• Significant experience in clinical triage and assessment.
• Current NMC/HCPC registration.
• Advanced Clinical Practice qualification.
• Non-medical prescribing authority.
• Willingness to work onsite in Scunthorpe and cover occasional face-to-face clinics.

• Undertake autonomous telephone triage and clinical assessment.
• Provide safe management including prescribing within scope.
• Determine appropriate care pathways (telephone, face-to-face, GP review).
• Maintain accurate clinical records.
• Support team and practice administrator.
• Attend CPD and governance activities.
