Job summary

Strathmore Medical Practice in Wrexham is seeking an experienced Advanced Nurse Practitioner to work with our GP team. You will assess, diagnose and treat acute on‑the‑day presentations and conduct home visits where appropriate.

This role offers flexible, part‑time hours, opportunities to shape our Klinik triage system, and a supportive, training‑focused environment dedicated to high‑quality patient care.

Qualifications

  • Registered nurse with current NMC registration.
  • Advanced Clinical Practice qualification or equivalent experience.
  • Experience in general practice, urgent care or primary care settings.
  • Independent prescribing qualification desirable.

Responsibilities

  • Conduct home visits for housebound and clinically appropriate patients.
  • Manage on-the-day acute presentations.
  • Assess, diagnose and treat patients independently within scope of practice.
  • Support and manage triage requests through Klinik online consultation system.
  • Provide clinical advice and signpost patients to appropriate services.
  • Prescribe independently where qualified.
  • Maintain accurate and timely clinical records.
  • Collaborate with GPs, practice nurses and wider healthcare staff.

Job Summary

Working closely alongside our GP team, the successful candidate will play a key role in managing acute on-the-day demand and supporting patients within the community.

Main Duties of the Job
  • Conducting home visits for housebound and clinically appropriate patients.

  • Providing clinical advice and signposting patients to the most appropriate service.

Strathmore Medical Practice is a busy, friendly and forward-thinking GP Practice serving a diverse and varied patient population in Wrexham. We are committed to delivering high quality, patient-centred care and continuously strive to develop innovative ways of working to meet the needs of our community.

Currently our Clinical Team consists of: 4 Partner GPs, 4 Salaried GPs, 4 Practice Nurses, 2 Advance Nurse Practitioners, 2 HCA. We are also a Training Practice and normally support 2 GP Registrars. Non Clinical Team consists of Reception, Prescribing Team, Letter Readers and a Secretary.

Job Responsibilities

This is a new role within the Practice supporting the GP on the day with clinical triage and home visits.

Person Specification
Qualifications – Essential
  • We are looking for an experienced ANP who:
  • Is a registered nurse with current NMC registration.
  • Holds a recognised Advanced Clinical Practice qualification or equivalent experience.
  • Has experience working within general practice, urgent care or primary care settings.
  • Is confident in autonomous assessment, diagnosis and treatment of acute conditions.
  • Has excellent communication and clinical decision‑making skills.
  • Is able to effectively manage risk and prioritise workload.
  • Independent prescribing qualification desirable.
  • Experience of digital triage systems, particularly Klinik, would be advantageous although training can be provided.
Desirable
  • Knowledge of clinical system EMIS and DOCMAN
Experience – Essential
