We are seeking a highly motivated and experienced Advanced Nurse Practitioner to join our friendly and supportive primary care team. The successful candidate will work autonomously, providing high-quality clinical care to patients, including assessment, diagnosis, treatment, prescribing, and management of acute and chronic conditions.

Main duties of the job


  • Undertake comprehensive patient assessments, including history taking, physical examination, diagnosis, and treatment planning.

  • Independently manage patients presenting with undifferentiated and complex conditions.

  • Prescribe medications within scope of practice and current legislation.

  • Request, interpret, and act upon diagnostic investigations.

  • Manage acute presentations and minor illnesses.

  • Support the management of patients with long-term conditions.

  • Provide health promotion, preventative care, and patient education.

  • Make appropriate referrals to other healthcare professionals and services.

  • Maintain accurate and contemporaneous clinical records.

  • Contribute to clinical governance, quality improvement, and service development initiatives.

  • Provide mentorship and support to nursing and healthcare colleagues where appropriate.

Job responsibilities


  • Assess, diagnose and manage patients presenting with undifferentiated and undiagnosed conditions.

  • Conduct comprehensive history taking and physical examinations.

  • Formulate differential diagnoses and management plans.

  • Request and interpret appropriate investigations.

  • Prescribe medications safely and effectively within scope of practice.

  • Manage acute and chronic illnesses in accordance with local and national guidelines.

  • Provide health promotion, preventative care and patient education.

  • Refer patients appropriately to secondary care and other healthcare professionals when required.

  • Recognise and respond appropriately to clinical emergencies.

  • Identify and manage risk while working autonomously and within professional boundaries.


Long-Term Condition Management


  • Support the management of patients with chronic diseases.

  • Contribute to care planning and monitoring of patients with complex needs.

  • Promote self-management and shared decision‑making.


Patient Care and Communication


  • Develop effective therapeutic relationships with patients and carers.

  • Ensure patients are involved in decisions regarding their care and treatment.

  • Communicate complex and sensitive information effectively and compassionately.

  • Advocate for patients and support equality of access to services.


Leadership and Service Development


  • Contribute to the development and implementation of clinical policies and procedures.

  • Participate in quality improvement initiatives, audits and service evaluations.

  • Support the development of innovative ways of working to improve patient care.

  • Act as a clinical role model within the practice.


Education and Training


  • Maintain professional competence through ongoing CPD and revalidation requirements.

  • Support and mentor nursing, healthcare assistant and administrative colleagues where appropriate.

  • Contribute to practice training programmes and educational activities.


Clinical Governance


  • Maintain accurate, timely and contemporaneous patient records.

  • Comply with professional, legal and ethical standards.

  • Participate in clinical audits and significant event reviews.

  • Work in accordance with safeguarding policies and procedures.

  • Ensure compliance with infection prevention and control standards.

  • Adhere to all practice, PCN and NHS policies and procedures.

Qualifications

Essential


  • Registered Nurse with current NMC registration.

  • Master's Degree in Advanced Clinical Practice or equivalent advanced practice qualification.

  • Independent/Supplementary Prescriber Qualification (V300).

  • Evidence of ongoing continuing professional development.


Desirable


  • Advanced Clinical Practice accreditation.

  • Minor illness and/or minor injury qualification.

  • Non-medical prescribing experience within primary care.

Essential


  • Significant post-registration nursing experience.

  • Experience working as an Advanced Nurse Practitioner, Advanced Clinical Practitioner, Emergency Nurse Practitioner, or similar advanced clinical role.

  • Experience of autonomous clinical assessment, diagnosis and management of patients presenting with undifferentiated and complex conditions.

  • Experience of prescribing independently within scope of practice.

  • Experience managing patients with acute illnesses in a primary care, urgent care, community or emergency care setting.

  • Experience requesting, interpreting and acting upon diagnostic investigations.

  • Experience working within a multidisciplinary team.

  • Experience maintaining accurate clinical records using electronic patient record systems.

  • Demonstrable experience of evidence-based practice and clinical decision-making.

  • Advanced clinical assessment and diagnostic skills.

  • Excellent communication and interpersonal skills.

  • Ability to work independently and manage clinical risk appropriately.

  • Strong organisational and time-management skills.

  • Ability to prioritise workload in a busy clinical environment.

  • Working knowledge of current NHS, NMC and primary care guidelines.

  • Commitment to continuous professional development and lifelong learning.
