Job summary

Job description

Law Medical is seeking to recruit an Advanced Nurse Practitioner to join its multidisciplinary primary care team. The role will involve delivering high-quality, patient-centred clinical care, supporting the assessment, diagnosis, and management of a wide range of acute and long-term conditions, while working collaboratively with GPs and other healthcare professionals.

The Advanced Nurse Practitioner will work autonomously within their scope of practice, contributing to clinical decision‑making, prescribing, and care planning. The post holder will also play an active role in supporting the wider multidisciplinary team and contributing to the ongoing development and objectives of the practice.

Main duties of the job

As an Advanced Nurse Practitioner, you will work autonomously within your scope of practice to deliver high-quality, patient‑centred care. Key responsibilities include managing acute and chronic conditions, conducting health assessments, running specialist clinics, prescribing medications, and promoting health and disease prevention. You will also contribute to clinical policy development, support safeguarding, and play an active role in clinical audits and service improvement across the practice.

Job responsibilities

ClinicalResponsibilities:

In accordance with the practice timetable, the ANP will workautonomously, delivering care within their scope of practice to the patientpopulation. Contributing towards clinical areas such as health promotion,chronic disease management, health prevention, well‑woman and well man clinicsas well as supporting the management team in the reviewing of clinical policyand procedure.

• Manage patients presenting with a range of acuteand chronic medical conditions, providing subject matter expert advice topatients
• Implement and evaluate individual specialisedtreatment plans for chronic disease patients
• Identify, manage and support patients as risk ofdeveloping long-term conditions, preventing adverse effects on the patientshealth
• Provide advanced, specialist nursing care topatients as required in accordance with clinical based evidence, NICE and theNSF
• Provide wound care (ulcer /Doppler etc.) topatients
• Provide specialist clinics such as earsyringing, contraception etc.
• Undertake the collection of pathologicalspecimens
• Request pathology services as necessary
• Process and interpret pathology and other testresults as required
• Provide chronic disease clinics, deliveringpatient care as necessary, referring patients to secondary / specialist care asrequired
• Maintain accurate clinical records inconjunction with extant legislation

Experience

• Experience working in a primary care environment;
• Experience in chronic disease management;
