Job summary

Qualifications

• Extensive experience in managing complete episodes of care including assessment, diagnosis, treatments and prescribing.
• Masters degree in Advanced Practice or equivalent post-graduate qualification and full accreditation/portfolio.
• Current UK professional registration with NMC or HCPC or equivalent.

Responsibilities

• Deliver advanced and complex clinical care including assessment, diagnosis, treatment and prescribing for acute, chronic and complex conditions.
• Work independently within scope of practice to manage complete clinical episodes using advanced diagnostic skills.
• Manage patient cases from initial assessment to conclusion with follow-ups and referrals.
• Utilise prescriptive authority following current guidelines to improve outcomes.
• Undertake home visiting when required, including triage of visits.

Job summary

We are seeking a highly skilled and autonomous Advanced Nurse Practitioner (ANP) to join our Team for 33 hours a week. This role is pivotal in providing high-quality, patient-centred care in a fast-paced, supportive general practice environment.
As an ANP, you will lead on delivering undifferentiated primary care to a varied patient population, working independently but with access to clinical support from the practice team. The successful candidate will demonstrate excellence in clinical assessment, decision-making, and prescribing, with a commitment to upholding the standards of care and safety expected by Mayflower Medical Group.

Main duties of the job

Key responsibilities include the application of advanced clinical skills to assess, diagnose, treat, and advise patients face to face, ensuring the use of evidence-based practice. The role encompasses the following areas:

Patient Care

Deliver advanced and complex clinical care, providing assessment, diagnosis, treatment, and prescribing for patients with acute, chronic, and complex conditions.

Autonomous Practice

Work independently within the scope of practice to manage complete clinical episodes, using advanced diagnostic skills and clinical judgment.

Case Management
