Advanced Nurse/Advanced Clinical Practitioner

An exciting new opportunity hasarisen for an experienced Advanced Nurse Practitioner or Advanced Clinical Practitionerto join our friendly team at Barton House MedicalPractice. 20 hoursper week, covering a Monday and Tuesday. We are recruiting due to retirement of our current ANP and we have another excellent ANP who covers the other days (triage run by GPs on a Friday). We are looking at a potential start date of April/May 2026.

Main duties of the job

We have recently instigated a platformof total triage for all online consultations. The ANP/ACP will actautonomously within their professional scope of practice providing care forpatients face to face, telephone consultations and have the main responsibility of triaging all online requests. Thesystem is urgency coded and the ANP/ACP will triage the requests into anappointment slot and manage any suitable and urgent cases. This improvesaccess for patients as they are allocated appointments based on clinicalneed.. All appointment requests come in via this system, whether urgent or routine and full training will begiven regarding the new triage system to the ANP/ACP.

It is a busy but enjoyable role and a fantastic teamenvironment. The post holder will operate withintheir own sphere of competence and agreed acceptable limits of practice toprovide expert professional clinical advice to patients, carers and colleagues,ensuring clinical safety and excellence.

The Advanced Nurse Practitioner/or Advanced Clinical Practitioner willneed to prioritise and triage the needs of patients accordingly, making anynecessary referrals for investigations in the appropriate manner. The postholder will use skills, knowledge and competencies as a qualified AdvancedNurse Practitioner/or Advanced Clinical Practitioner in order to be responsible and accountable for managingpatient caseloads for treatments, referral, admission and discharging patientsas appropriate.

Job responsibilities
Clinical Nursing Practice
  • Work aspart of a multi-disciplinary team to deliver safe and high quality care.
  • Carriesout clinical nursing practice at an advanced level using expert clinicalskills, critical judgements & knowledge to deliver appropriate and safecare.
  • Deliveryof care in line with NSF, NICE guidelines and evidence-based care
  • Triage andtreat patients, performing specialist assessment of their needs and makereferrals as necessary to other members of the primary and secondary healthcare teams as appropriate.
  • Co-ordinatethe planning and delivery of care ensuring patients and carers/relatives areinvolved.
  • Prescribingand reviewing medication that is appropriate to patient needs and in accordancewith latest evidence-based practice, national and practice protocols and withinthe scope of qualified practice.
  • Diagnoseand manage acute and chronic conditions, including drug and non-drug basedtreatment methods into a management plan.
  • Requestdiagnostic investigations and interpret within agreed protocols e.g. x-ray,blood tests, scans, etc.
  • Assess,diagnose and treat patients in practice premises who require medical attentionand/or refer to appropriate agency.
  • Referspatients to other members of the multi-disciplinary practice teams, secondarycare organisations and other organisations as necessary.
Care Management
  • Communicatesconfidential and sensitive information to patients, relatives and carers inrelation to their condition.
  • Providehighly specialised care advice beyond the scope of normal Nursing practice e.g.care treatment plans, making referrals as appropriate (including fast trackreferrals).
  • To monitorand meet care outcomes against standards and recommend or initiate changes asnecessary.
Health Improvement
  • Managementof health screening programmes and ensures other team members are aware ofhealth priorities and screening objectives and programmes.
  • Innovationin practice to meet ongoing demands on the practices delivery of care.
  • Improvehealth outcomes for patients by introducing new developments, and candemonstrate them as reports and presentations where appropriate.
  • Supportspractitioners in reporting the practice population to promote healthimprovement and disease prevention.
  • Makesreferrals to appropriate agencies (statutory and voluntary) where necessary
Providing assessment, screening services, health education advice

These mayinclude (but not restricted to);

  • Bloodpressure checks, Urinalysis, Dressings, Contraception, Routine immunisationsand vaccinations, Removal of sutures / clips,First registration checks / health surveillance / general health advice,Organise and plan the provision of nursing services for the practice to includethe delegation of duties to other members of the practice nurse team asappropriate
Administration and professional responsibilities
  • Produceaccurate, contemporaneous and complete records of patient consultation,consistent with legislation, policies and procedures
  • Participatein the administrative and professional responsibilities of the practice team
  • Accurateand legible notes of all consultations and treatments are recorded in thepatients notes
  • Ensureclinical systems kept up to date, recording and/or amending accurate details
  • Ensureappropriate items of service claims are made accurately, reporting any problemsto the practice administrator
  • Ensureaccurate completion of all necessary documentation associated with patienthealth care and registration with the practice
  • Attend andparticipate in practice meetings as required
Training and personal development
  • Maintainup to date skills and knowledge, maintaining awareness of professional issuesat an advanced level.
  • Trainingneeds will be monitored by yearly appraisal and will be in accordance withpractice requirements. Personal development will be encouraged and supported bythe practice.
  • Assesseffectiveness of care delivery through self-reflection and peer review, as wellas benchmarking and formal evaluation.
  • Contributeto the identification and assessment of learning needs of staff and otherprofessionals and assist in planning effective programmes of education.
  • Takingresponsibility for own development, learning and performance and demonstratingskills and activities to others who are undertaking similar work and ensure owneducational commitment is at least sufficient to maintain revalidationrequirements.
Developand maintain a Personal Learning Plan
  • Regularlyparticipate in clinical supervision.
