Job summary

NHS Lanarkshire is seeking an experienced Nurse, Midwife or Allied Health Professional to join the Frailty Network as an Advanced Specialist Practitioner. You will lead multidisciplinary teams, manage a highly specialist caseload, apply evidence-based practice, and drive quality improvement across community hospitals in Lanarkshire.

You will provide expert clinical leadership, develop and evaluate frailty pathways, and collaborate with Health and Social Care Partnerships to deliver

Qualifications

• Educated to/working towards/operating at Masters Level.
• Postgraduate expertise within speciality / area of practice.
• Comprehensive knowledge of clinical guidelines and standards.

Responsibilities

• Lead or manage a programme of quality improvement and clinical audit.
• Lead supervision, education and development of the multidisciplinary team.
• Provide expert advice and support within a defined area of practice.
