Job summary

Royal Victoria Hospital in Dundee, under NHS Scotland, seeks an Advanced Practice Dietitian to work within the Frailty at Home Service. You will provide specialist dietetic assessment and intervention for people with complex nutritional needs, across community and hospital settings, improving continuity of care.

You will lead on service development, quality improvement, and data-driven planning, supporting frailty pathways and timely discharge to prevent avoidable hospital admissions.

Qualifications

• Significant post-registration experience as a specialist dietitian.
• Experience in service development, quality improvement, audit or service evaluation.
• Experience collecting, analysing and presenting outcome data to support service improvement.

Responsibilities

• Provide specialist dietetic assessment and intervention for people with complex nutritional needs.
• Lead service evaluation and quality improvement activity.
• Support integrated frailty pathways with in-reach to acute services and outreach to the community.
• Influence the future delivery of frailty services and develop the model of care.
