Job summary

Qualifications
- Office-agnostic professional registration and advanced practice competencies.
- Postgraduate qualification requirements for ACP/advanced clinical roles.
- Ability to complete or undertake MSc via apprenticeship pathway.
Responsibilities
- Practice as highly specialist clinician for complex health conditions at home.
- Offer holistic assessment and treatment to maximise wellbeing and reduce hospital visits.
- Work to full scope of practice to achieve optimal outcomes.
- Support CPD and competencies relevant to the field.
- Provide clinical triage and assessment support for Urgent Community Response and Virtual Wards.
