Job summary

The Methodist Church in Britain is seeking an Advanced Clinical Practitioner to join the Out of Hours service. The role focuses on assessing, diagnosing, and managing complex presentations, including prescribing within approved protocols.

Staff work across multiple sites in the region with emphasis on autonomous practice and high standards of clinical care. Applicants should hold an MSc in Advanced Clinical Practice, Full NMC registration with no restrictions, and Non-Medical Prescriber V300.

Qualifications

• Autonomous working in urgent/primary care settings is essential.
• Experience in handling complex undifferentiated presentations in a clinical setting.
• Proven ability to diagnose and manage acute and chronic conditions within protocols.

Responsibilities

• Assess and diagnose patients with undifferentiated and complex needs.
• Examine patients clinically from physiological and psychological perspectives.
• Plan, implement and evaluate care for individuals with long-term conditions.
• Prescribe and review medications according to evidence-based protocols.
• Support adherence to prescribed treatments and monitor effectiveness.
• Provide guidance on medications and potential interactions.

Job description

This permanent Advanced Clinical Practitioner post sits in the Out of Hours department of Primary Care 24, a not-for-profit social enterprise that mainly delivers NHS contracts. The work centres on assessing and diagnosing undifferentiated and complex presentations, then planning and managing acute and chronic conditions, including prescribing. Full and part time positions are available across several sites in the region, and experience of working autonomously is essential. Fit depends on full NMC registration, non-medical prescribing and Level 7 clinical examination and diagnostics.

What the job involves

• Assess, diagnose, plan, implement and evaluate care for patients with undifferentiated, undiagnosed and complex needs
• Clinically examine patients from physiological and psychological perspectives and plan their care
• Identify, diagnose and manage treatment plans for patients at risk of long‑term conditions
• Diagnose and manage acute and chronic conditions using drug and non-drug treatments
• Prescribe and review medication for effectiveness within scope of practice and evidence‑based protocols
• Support patients to comply with and adhere to prescribed treatments
• Advise on prescribed and over‑the‑counter medication, side‑effects and interactions
