Job summary

The Blandford Group Practice in Blandford Forum is seeking an experienced Advanced Clinical Practitioner to join our CQC Outstanding primary care team. The role offers 30 hours per week, with Wednesday to Friday shifts, delivering patient care from triage through to prescribing within own professional boundaries.

You will work closely with GPs and the wider clinical team, support the on‑the‑day urgent clinical need via the Triage Hub, and lead on safe clinical decision‑making, audits and quality

Qualifications

Registered first level nurse with UK registration.
Post-registration nursing experience in primary or community care.
Experience in nurse-led triage and management of minor illness/injuries.
Experience in leadership and quality improvement initiatives.

Responsibilities

Assess patients with acute illness or chronic disease; plan and review care.
Undertake home visits for acute illness.
Contribute to triage team assessing and signposting requests.
Prescribe and review medications according to guidelines.
Lead and mentor team; maintain patient records.
Coordinate care to improve patient outcomes.
