Job summary

The Blandford Group Practice in Blandford Forum is seeking an experienced Advanced Clinical Practitioner to join our CQC Outstanding primary care team. The role offers 30 hours per week, with Wednesday to Friday shifts, delivering patient care from triage through to prescribing within own professional boundaries.

You will work closely with GPs and the wider clinical team, support the on‑the‑day urgent clinical need via the Triage Hub, and lead on safe clinical decision‑making, audits and quality

Qualifications

• Registered first level nurse with UK registration.
• Post-registration nursing experience in primary or community care.
• Experience in nurse-led triage and management of minor illness/injuries.
• Experience in leadership and quality improvement initiatives.

Our CQC Outstanding rated Practice is looking to recruit an Advanced Nurse Practitioner to join our friendly and welcoming team. Our ACP Team is essential to the delivery of our service, working closely with our GP's and wider clinical team to support our patients health needs. We utilise a Triage Hub model that supports patients to submit digital clinical requests, and this post holder will support this work by responding to on the day urgent clinical need.

This role will be for 30 hours, Wednesdays, Thursdays and Fridays 8:30am to 6:30pm, with Mondays and Tuesdays as day off!

Please note that the hourly rate is non negotiable.

Main duties of the job

As an Advanced Clinical Practitioner at the Blandford Group Practice you will be an experienced nurse who, acting within their professional boundaries, will provide care for patients presenting at the practice from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. You will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will also demonstrate critical thinking in the clinical decision-making process and be a registered prescriber.

Job responsibilities

• Assess patients presenting with acute illness or for review of chronic disease conditions.


• Undertake home visits for acute illness


• Play a part in our triage team to assess and sign post patient requests received via SystmConnect.


• Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.

• Work with patients and their carers in order to support compliance with and adherence to prescribed treatments.


• Provide information and advice on prescribed or over‑the‑counter medication on medication regimens, side-effects and interactions.
