A leading healthcare provider in Camberley is seeking an experienced Advanced Clinical Practitioner (ACP) to enhance general practice capacity within a collaborative primary care team. The role involves managing urgent and same-day appointments, requiring strong clinical skills and the ability to work autonomously. Candidates should possess relevant qualifications and experience in primary care, with a commitment to delivering high-quality patient care.
This role offers an exciting opportunity for an experienced Advanced Clinical Practitioner (ACP) with prescribing skills to join our Primary Care Network (PCN). You will support local GP Practices by providing additional patient access on Saturdays and weekday evenings, working across a rotating schedule among 7 GP Practices.
This is a flexible role; you will work under a Bank contract and need to commit to clinics with at least 3 weeks' notice. Your work will mainly focus on urgent and same-day appointments, with some routine general practice appointments depending on the PCN/practices' needs and your skills and experience.
You will enhance core general practice capacity as part of a collaborative primary care team supporting residents in Surrey Heath. You should be experienced in general practice, able to work autonomously, managing a caseload for urgent and on-the-day care with minimal supervision from onsite GPs under a Primary Care Consultant model. Skills in urgent care, minor illness management, and support for patients with long-term conditions are essential.
You will manage an identified caseload or patient cohorts, providing comprehensive care from history taking and clinical assessment to diagnosis, treatment, and evaluation, demonstrating safe and evidence-based clinical decision-making.
