Job summary

This role is an exciting opportunity for an experienced paediatric advanced clinical practitioner (PACP) with existing prescribing skills to join our Primary Care Network (PCN). You will support our local GP Practices with additional patient access on Saturdays and weekday evenings, working across our 7 GP Practices.

You will work under a Fixed Term Contract. Your work will primarily focus on urgent and on-the-day appointments, with some routine general practice appointments depending on the PCN/practices' needs and your skills and experience.

Main duties of the job

You will support core general practice capacity and work as part of a positive, proactive, and collaborative primary care clinical team supporting children aged 0-17 years in Surrey Heath. You will be experienced in general practice, able to work autonomously, managing a caseload of booked appointments for urgent and on-the-day care needs, with minimal supervision from onsite GPs under a Primary Care Consultant model. You will need skills in providing urgent care, managing minor illnesses, and supporting children with long-term conditions.

You will expertly manage an identified caseload or cohort(s) of patients and presenting complaints; providing comprehensive care from initial history taking, clinical assessment, diagnosis, treatment, and evaluation, demonstrating safe, competent clinical decision-making and evidence-based care.

About us

We are Surrey Heath Community Providers Limited, a federation of 7 GP practices across 10 sites, covering over 97,000 patients in Surrey Heath. Established in 2016, we employ around 200 staff across clinical and support teams. Since 2019, we have hosted the Primary Care Network in Surrey Heath.

As a GP Federation and Primary Care Network, we are committed to representing our practices and championing primary care by working with local general practices and system partners to provide community-based healthcare services. We are dedicated to safe, compassionate care and continuous quality improvement for our patients.

Patients are at the heart of everything we do, and we ensure they feel safe, supported, communicated with, and respected, especially during vulnerable times.

Our vision is to deliver high-quality, seamless healthcare that enables people to lead healthier lives while feeling supported and cared for.
