Job summary

gtd healthcare is seeking an Advanced Clinical Practitioner for New Bank Health Centre in Manchester. The role offers a salary range of £58,991 to £71,281 per year, with NHS Pension and yearly salary increments.

You will join a multidisciplinary team and be supported on site by Salaried GPs, Medical Lead, Nurses and Administration Teams. You will work with GP's, Practice Nurse, Pharmacist and Physician Associate, contributing to QoF targets, CQC objectives and ongoing professional development

• Registered professional with MSc in Advanced Clinical Practice or equivalent.
• V300 Non-Medical Prescriber (gateway dependent).
• Experience as a senior clinician and in leading quality improvement.
• Experience in primary or unscheduled care settings.

Advanced Clinical Practitioner - New Bank Health Centre

Advanced Clinical Practitioner, £58,991 to £71,281 dependent upon experience, yearly salary increments, NHS Pension.

gtd healthcare is one of the largest NHS commissioned Primary Care, Out of Hours & Urgent Care providers in the North West of England. We have a fantastic opportunity for an Advanced Clinical Practitioner to work at New Bank Health Centre Service.

New Bank Health Centre, part of gtd healthcare, is a practice located in Longsight, Manchester. We are situated on Stockport Road in a purpose built medical centre which offers great links to public transport.

As an Advanced Clinical Practitioner, you will work alongside a supportive multidisciplinary team who have a real commitment to continuous improvement, shared learning and best practice, while being passionate about providing patients with the highest possible quality care.

You will be supported on site by our Salaried GPs, Medical Lead, Nursing and Administration Teams, along with Practice Management and the widergtd Team.

Main duties of the job

Using EMIS, you will consult via telephone, video or face to face, according to patient needs. (Full training and support for remote consultations will be available if required). You will be supported by the non clinical team who have a good knowledge of our extensive local hospital and community services.

The clinical team includes GP's, Practice Nurse, Pharmacist, Physician Associate and HCA. You will possess leadership qualities and provide support to the wider team, act as a positive role model and proactively identify areas for quality improvement. You will proactively contribute to improving health outcomes for patients by identifying new initiatives / ways of working and as such promote health improvement and disease prevention.

In addition, you will assist to further improve standards of care and, achieve QoF targets as well as KLOE objectives, as outlined by CQC.

You will have full access to our gtd learning academy and clinical forums, including the NMP monthly forum, which is available to all our staff. This provides a great opportunity to grow and expand your clinician skills/knowledge and to excel as an ACP. You are also allocated CPD time weekly.

The extensive footprint of the organisation provides you with an opportunity to be exposed to a wide variety of different services such as urgent care, specialist services and out of hours working, where shared learning is encouraged.

Job responsibilities

Clinical Practice

• Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
• Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
• Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
• Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
• Refer and discharge safely ensuring that safety-netting advice is offered.
• Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.

Communication

• Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
• Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
• Anticipate barriers to communication and take action to improve communication.
• Maintain effective communication within the organisation and with external stakeholders.
• Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

Non-Medical Prescribing

• If you are employed in a prescribing capacity, you must only prescribe within your scope of practice and comply with the gtd healthcare NMP governance registration processes.
• You must maintain your knowledge and skills and prescribe within your clinical competencies.
• The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance.

Delivering a Quality Service

• Recognise and work within own competence and professional code of conduct as regulated by the NMC/HPC etc. Understand own role and scope and identify how this may develop over time.
• Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
• Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
• Ensure professional revalidation requirements are met.
• Deliver care according to NSF, NICE guidelines and evidence-based care.
• Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
• Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities.
• In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.
• Support and participate in shared learning across the organisation and wider organization.
• Understand and apply guidance that supports the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patient health procedures and local guidance.

Qualifications

• Registered first level nurse, paramedic, pharmacist or other AHP with MSc Advanced Clinical Practice or credentialing qualification or equivalent. V300 Non-Medical Prescriber (gateway dependent) dependent on professional eligibility.
• Mentor/Teaching qualification. Clinical supervision training and experience.
