Job description

Are you passionate about providing exceptional care to the elderly? Do you thrive in a collaborative, multidisciplinary environment? Join our dedicated team at Mansfield Community Hospital as an Advanced Clinical Practitioner (ACP) and make a real difference in the lives of frail, elderly patients.

As an ACP, you will be at the heart of our geriatric non-acute/rehabilitation wards, working closely with doctors, nurses, allied health professionals, and support staff. Your role will be crucial in ensuring that our patients receive the highest quality care as they transition from acute medical wards at Kings Mill Hospital to their homes.

You will have the autonomy to make complex clinical decisions, deliver advanced therapeutic interventions, and lead clinical practices. This position offers a unique blend of clinical excellence, leadership, and continuous professional development. With no on-call commitments and a supportive out-of-hours service, you can focus on what you do best—providing outstanding patient care.

You can be part of a team that values excellence, collaboration, and continuous improvement in a supportive and educational environment. Apply today and take the next step in your career as an Advanced Clinical Practitioner!

Key Responsibilities:

  1. Deliver high-quality clinical assessment, intervention, and treatment for a diverse patient population.
  2. Make complex clinical decisions autonomously, based on thorough assessments and diagnostic interpretations.
  3. Provide expert care within your professional scope, ensuring the best outcomes for patients.
  4. Work within the Multi-professional Framework for Advanced Clinical Practice in England.
  5. Collaborate with clinical teams to enhance patient care, develop services, and maintain clinical excellence.
  6. Lead and support medical, nursing, and allied health teams, contributing to the Trust's advanced practice strategy and vision.
  7. Engage in training and education within the clinical area, fostering a culture of learning and development.
  8. Maintain your professional competence and pursue opportunities for research and audit.
  9. Participate in formal and informal educational activities, including weekly medical Grand Rounds, geriatric department meetings, and specialist clinics.
