We provide community-based frailty care focusing on treating people in the community where possible, such as our Community Diagnostic Centre in partnership with the Acute Trust.

Job Summary: The post holder will work under direct or indirect supervision as a trainee advanced clinical practitioner, delivering high-quality care, making clinical decisions based on advanced skills, and developing across all four pillars of advanced clinical practice: research, leadership, clinical practice, and education, as outlined by Health Education England (2017).
The post holder will build evidence across these pillars, provide clinical leadership and support to the practitioner workforce, nursing, and allied health teams to improve patient experience, and support service delivery with innovation and ongoing clinical care, ensuring patients receive the right care at the right time.

Salary: £48,055 to £54,991 during training & £55,976 to £63,004 once qualified
