Job summary

HUC provides the NHS 111 and Out of Hours GP service to over 4.5 million patients across Hertfordshire, Cambridgeshire, Bedfordshire, West Essex, and the South West of England. We are an exciting and supportive organization with extensive expertise, ensuring optimal outcomes for every caller.

We are seeking highly skilled, motivated ACPs to join our Clinical Assessment Service (CAS), supporting patients 24/7 across our network.

Working remotely or from one of our contact centres, you will conduct clinical assessments via telephone and video using advanced digital platforms. Our focus is on reducing unnecessary emergency department visits and ambulance dispatches by providing safe, effective, and timely clinical management. You will be part of a multidisciplinary team including GPs, ACPs, ENPs, ECPs, Urgent Care Practitioners, and operational staff.

Main duties of the job
  • Assess, diagnose, treat, and discharge patients with minor illnesses and injuries via telephone and video consultation.
  • Manage minor illness, injury, ENT, and ophthalmic presentations, providing appropriate care or onward referrals.
  • Prescribe medication electronically and refer patients to primary, secondary, community, or emergency services as needed.
  • Deliver health education advice, promote self-care, and support informed health decisions.
  • Recognize acutely ill patients and initiate urgent interventions or escalate appropriately.
  • Identify safeguarding concerns and make referrals per policies and best practices.
  • Maintain accurate, clear, and contemporaneous clinical records, contributing to audits, service improvements, and KPIs.
  • Engage in professional development, attend meetings, and adhere to governance and clinical standards.
