Job summaryWe are delighted to offer an opportunity for an Advanced Clinical Practitioner (ACP) to join medical Same Day Emergency Care (SDEC) within the Directorate of Urgent and Emergency Care at Oldham Care Organisation. This is a permanent, full time position 37.5 hours per week). An MSc in Advanced Clinical Practice is essential, however we welcome applications from newly qualified, almost qualified and experienced ACP's who have a real passion in admission avoidance.
SDEC boasts a diverse team of ACP's who are supported by medical and nursing staff. Autonomously assessing, investigating and managing same day emergency medical patients will be your primary role; also, you will in-reach into the Emergency Department (ED) as well as supporting referrals from GP's and NWAS. The ACP team together with the consultant Clinical Lead, the nursing team and clerical staff are committed to the ongoing success of SDEC through innovative pathway development and collaborative working, both in secondary and primary care. Essentially, we strive to ensure the right person receives access to the right care at the right time, by the right person.

Main duties of the jobYour clinical caseload will be diverse and case management should be underpinned by a dedicated approach to patient-centred care and collaborative working alongside our multi-disciplinary teams, to ensure a tailored management plan is put in place which addresses both current issues and proactively looks to prevent potential issues which may arise. Sound clinical reasoning and risk management are key skills required to ensure that we can safely avoid admissions and continue to monitor patients via an ambulatory pathway.
