We are looking for a motivated and experienced Advanced Nurse Practitioner or Advanced Clinical Practitioner to join our friendly and progressive team at Pendleside Medical Practice. This is a fantastic opportunity to work in a supportive, well-established training practice, providing high-quality care to our patient population.

A role based in beautiful Clitheroe, serving over 11,800 patients
Working autonomously within your professional scope, you'll provide care across the full patient pathway from history taking and clinical assessment to diagnosis, treatment and follow-up. You'll handle acute, same-day care via face-to-face and telephone consultations, and contribute to chronic disease management, proactive care planning, and liaison with external partners.
We are especially interested in candidates with a passion for palliative care and supporting care home residents. A portion of this role will involve conducting regular ward rounds at our three local care homes.
Pendleside Medical Practice is a well-respected, training GP practice located in the picturesque market town of Clitheroe. Our team includes 7 GP Partners, 3 Salaried GPs, 1 ANP, 2 Physician Associates, and excellent nursing and admin teams. We pride ourselves on a culture of clinical excellence, teamwork, and professional development.
We are a key part of the Ribblesdale PCN, collaborating with nearby practices to improve local health outcomes.
Clinical Nursing Practice
Delivery of care in line with NSF, NICE guidelines and evidence-based care
Coordinate the planning and delivery of care ensuring patients and carers/relatives are involved.
Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.
Care Management
Communicates confidential and sensitive information to patients, relatives, and carers in relation to their condition.
Provide highly specialised care advice beyond the scope of normal Nursing practice, e.g., care treatment plans, making referrals as appropriate (including fast track referrals).
Contribute to local and national practice targets and clinical remit, e.g., QOF, prescribing incentive scheme, and national benchmarking.
Monitor and meet care outcomes against standards and recommend or initiate changes as necessary.
Health Improvement
Manage health screening programmes and ensure other team members are aware of health priorities and screening objectives and programmes.
Innovate in practice to meet ongoing demands on the practice's delivery of care.
Improve health outcomes for patients by introducing new developments, and demonstrate them through reports and presentations where appropriate.
Support practitioners in reporting the practice population to promote health improvement and disease prevention.
Make referrals to appropriate agencies (statutory and voluntary) where necessary.
Provide assessment, screening services, and health education advice.
