We are pleased to offer an opportunity for an Advanced Clinical Practitioner (ACP) to join the Same Day Emergency Care (SDEC) team within the Directorate of Urgent and Emergency Care at Oldham Care Organisation. This is a permanent, full-time position (37.5 hours per week). An MSc in Advanced Clinical Practice is essential, but we welcome applications from newly qualified, nearly qualified, and experienced ACPs who are passionate about admission avoidance.
SDEC boasts a diverse team of ACPs supported by medical and nursing staff. Your primary role will involve autonomously assessing, investigating, and managing same-day emergency medical patients. You will also in-reach into the Emergency Department (ED) and support referrals from GPs and NWAS. The team, led by the consultant Clinical Lead, along with nursing and clerical staff, is committed to innovative pathway development and collaborative working across secondary and primary care, ensuring the right care at the right time by the right person.
