Main duties of the job

The postholder is an experienced clinician working within their professional boundaries to collaborate with the UTC team, providing comprehensive care from initial assessment to treatment and evaluation within the urgent care setting.

The role involves demonstrating safe clinical decision-making, managing NHS 111 direct booking calls, and providing clinical leadership in line with advanced practice pillars.

Job responsibilities
  • Assess, diagnose, plan, implement, and evaluate patient care for undiagnosed and complex cases (children and adults).
  • Conduct physiological and psychological assessments and plan accordingly.
  • Triage referrals and calls from clinicians, patients, and others into appropriate services.
  • Order and interpret investigations such as blood tests and X-rays.
  • Work autonomously within organizational protocols and professional scope.
  • Manage acute and chronic conditions, integrating medication and non-medication treatments.
  • Prescribe and review medications, ensuring therapeutic effectiveness.
  • Support patient adherence and health promotion strategies.
  • Maintain competence according to regulatory and revalidation standards.
  • Communicate effectively with patients, carers, and external stakeholders, including delivering bad news sensitively.
  • Coordinate care for complex patients navigating the healthcare system.
  • Promote the positive image of Tower Hamlets GP Care Group externally.
Person Specification
Experience
  • Experience working independently with urgent care patients.
Skills and Abilities
  • Ability to prescribe medications within scope of practice.
  • Ability to interpret blood tests and X-rays.
Qualifications
  • MSc in Advanced Practice or Dip He (N) Advanced Practice.
Knowledge
  • Knowledge of the 4 pillars of advanced practice.
Additional requirements

This role is subject to the Rehabilitation of Offenders Act 1975 (Exceptions Order). A DBS check is required. Applicants must hold current UK professional registration.
