Job summary

Pembroke Medical Group in Paignton is seeking an experienced nurse/paramedic for a Visiting Clinical Care Practitioner / Community Matron role. You will provide advanced care to frail and housebound patients at home, in care homes, and at the surgery.

You will work within a GP team to deliver high-quality primary care. The position requires independent prescribing where eligible, strong clinical decision-making, and a patient-centred approach.

Qualifications

• Proven ability to provide advanced clinical care in primary care settings.
• Experience in autonomous decision making and patient management.

Responsibilities

• Deliver high standard of patient care as an Advanced Care Practitioner in general practice.
• Provide a first point of contact for undiagnosed problems using history taking, examination and diagnosis.
• Where independent prescriber, prescribe safe and appropriate medication.
• Prioritise health problems and initiate emergency care when needed.
• Promote health and wellbeing through education, screening and communication.
• Maintain accurate records in line with GDPR and practice policies.
• Collaborate with GP team, nurses, pharmacists and other health professionals.
• Support carers and work across home, care home, and surgery settings.
