Pembroke Medical Group in Paignton is seeking an experienced nurse/paramedic for a Visiting Clinical Care Practitioner / Community Matron role. You will provide advanced care to frail and housebound patients at home, in care homes, and at the surgery.
You will work within a GP team to deliver high-quality primary care. The position requires independent prescribing where eligible, strong clinical decision-making, and a patient-centred approach.

We are looking for an enthusiastic, experienced nurse or paramedic to join us as a Visiting Clinical Care Practitioner / Community Matron. We have already seen fantastic results from our current Community Matron, who works primarily in local care homes. Because of this success, we are expanding the service to include our housebound patients, giving them better continuity, more personalised support, and high-quality care in the comfort of their own homes.

The Post-holder will be an experienced nurse/paramedic, who acting withing their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe, clinical decision making and expert care for patient withing the general practice.
The post-holder will provide this service to our care home residents, patients in their own homes and at the surgery.
The post-holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
The post-holder will build effective working relationships with other health care professionals.
The post-holder will work withing the team of GP's, nurses and health care assistants to deliver excellent primary health care services within the Practice.

The Pembroke Medical Group is part of the Baywide Primary Care Network, which includes Compass House Medical Centre in Brixham and Chilcote Surgery in Torquay. Together we serve a population of 50,000 patients. The expectation is that the successful candidate will collaborate with our GPs, Nurse Practitioners, Nursing Team and Pharmacists, as well as our colleagues in Community Nursing, Physiotherapy, Occupational Therapy, Social Care and at SWAST.

To deliver a high standard of patient care as an Advanced Care Practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.
Provide a first point of contact within the Practice for patients presenting withundifferentiated, undiagnosed problems, making use of skills in history taking, physicalexamination, problem-solving and clinical decision-making, to establish a diagnosis andmanagement plan.
Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
Where the post-holder is an independent prescriber: to prescribe safe, effective andappropriate medication as defined by current legislative framework.
To prioritise health problems and intervene appropriately, including initiation of effective emergency care.
Promote health and well-being through the use of health promotion, health education,screening and therapeutic communication skills.
To clearly communicate the health status of the patient using appropriate terminology,technology and format.
Provide safe, evidence-based, cost effective, individualised patient care.
To work within the Practice guidelines, policies and protocols.
