Job summary

Meeveem Limited in Paignton is seeking an experienced nurse or paramedic to join as Community Matron/Advanced Visiting Practitioner. You will provide high-quality care to frail or housebound patients in their homes and in the practice, working autonomously within professional boundaries.

You will assess, plan, and review treatments, prescribe where appropriate, and collaborate with the GP team to deliver personalised care.

Qualifications

• Essential nursing qualifications and advanced practice credentials.
• Ability to manage complex patient needs within primary care.
• Experience with prescribing and clinical governance in community settings.

Responsibilities

• Assess, diagnose, plan, implement and evaluate treatment for patients with undifferentiated or complex needs.
• Provide care to care home residents, housebound patients, and patients at the surgery.
• Collaborate with multi-disciplinary team to meet patient needs and deliver policy.
• Build and maintain relationships with other health care professionals.
• Deliver high-quality primary health care services in the Practice.

Education

Registered nurse with MSc or equivalent

Relevant nursing/health degree

Mentor, teaching qualification

Clinical supervision training and experience

Extended independent non-medical prescribing

Advanced clinical knowledge - 4 pillars of advanced practice

Management of patients with complex needs

Accountability of own role and other roles

Local and national health policy

Wider health economy
