Job summary

Boston Primary Care Network in the United Kingdom invites an experienced Advanced Nurse Practitioner to join the Home Visiting Wellbeing Team, delivering patient-centered care in and around Boston.

You will assess and manage acute and minor illnesses during home visits, working with GP practices as part of Boston PCN. Countrywide guidelines apply; driving licence and UK registration are essential.

Qualifications

• Registered with NMC as RGN and evidence of NMP qualification or working towards, including advanced examination skills.
• Experience in a demanding primary care clinical setting is preferred.
• Independent prescriber status is required or progression towards it.

Responsibilities

• Assist daily with home visits including triage and seeing on the day patients for acute and minor illness.
• Care for housebound and care home residents in the community on behalf of Boston PCN practices.
• Work within a diverse, multi-disciplinary team to provide high-quality care in patients' homes.
