Meeveem Limited in Paignton is seeking an experienced nurse or paramedic to join as Community Matron/Advanced Visiting Practitioner. You will provide high-quality care to frail or housebound patients in their homes and in the practice, working autonomously within professional boundaries.

Responsibilities

We are looking for an enthusiastic, experienced nurse or paramedic to join us as a Community Matron/Advanced Visting Practitioner. If you love building relationships, enjoy working autonomously, and want to make a real difference to frail or housebound patients, we would love to hear from you.

We have already seen fantastic results from our current Community Matron, who works primarily in local care homes. Because of this success, we are expanding the service to include our housebound patients too giving them better continuity, more personalised support, and high-quality care in the comfort of their own homes.

Main duties of the job
  • The post-holder will be an experienced nurse/paramedic, who acting within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patient within the general practice.
  • The post-holder will provide this service to our care home residents, patients in their own homes and at the surgery.
  • The post-holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
  • The post-holder will build effective working relationships with other health care professionals.
  • The post-holder will work within the team of GPs, nurses and health care assistants to deliver excellent primary health care services within the Practice.
About us

Operating from our main site in Paignton and our branch site in Torquay we care for upwards of 25,000 patients in an organised and strong team environment. This means meeting for coffee every day, enjoying our practice funded socials and being supportive to new members of our team. Our 22 GPs are currently supported by ANPs, Pharmacists, Paramedics and ARRS roles staff including Pharmacy Technicians, and Social Prescribers as well as large and experienced nursing, reception, and admin teams.

Job responsibilities
  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
  • Work with patients in order to support compliance with and adherence to prescribed treatments Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health Promote and deliver evidence-based care for patients presenting with aural conditions
