Job summary

Cambridgeshire County Council in Cambridge is seeking a qualified social worker to join the Adult Early Help Team on a full-time permanent basis. The role focuses on prevention and early intervention, promoting independence and wellbeing for adults aged 18 and over, with duties including screening referrals, developing community action plans, and coordinating with unpaid carers.

Qualifications

• Qualified Social Worker with current Social Work England registration.
• Good knowledge of the Care Act 2014 and Mental Capacity Act 2005.
• Experience working in a multidisciplinary environment.
• Strong communication skills and confident telephone manner.
• Able to plan, organise and work under pressure to deadlines.

Responsibilities

• Triaging referrals and screening for the Adult Early Help duty function.
• Support team colleagues in a front door role and pathway development.
• Develop community action plans and support unpaid carers.
• Contribute to safeguarding referrals and risk identification.
• Apply Mental Capacity Act principles in decision making.
• Use strengths-based approaches and connect with community resources.
• Collaborate with Adult Social Care, Health services and Prevention teams.
