Qualifications

• ASYE evidence required
• Up-to-date knowledge of strengths-based social work with adults including safeguarding
• Knowledge of health & social care legislation and policy context
• Strong interpersonal and communication skills
• Ability to use Microsoft and electronic social care records databases
• Self-starter, well-organised with a commitment to high-quality, person-centred services
• Fluency in English is required for this role

Responsibilities

• Provide statutory support to adults with care needs and their carers/families, including information, assessment, support planning and safeguarding
• Comply with legal frameworks for social care with personalisation principles
• Work within defined budgets for social care
• Undertake Mental Capacity Assessments and Best Interest decisions
• Collaborate with primary, secondary and private providers to coordinate support for individuals and carers
• Signpost/advocate for people, highlighting local voluntary and community services
