Job Title: Adult Social Care Team Manager - Safeguarding Team

Responsibilities

• Provide leadership and support to your team, driving them to deliver a high-quality service that promotes healthy and independent living for adults.
• Foster the development of team members, ensuring they have the resources and guidance needed to excel in their roles.
• Offer expert advice and support on complex safeguarding cases, ensuring compliance with legislative and organisational standards.
• Act as an escalation point for complaints, handling feedback in a way that maintains and builds positive relationships with service users.
• Implement integrated working approaches with partners, communities, and the voluntary sector, achieving successful outcomes for adults.
• Represent the team in various meetings, both internally and with partner agencies, to ensure cohesive and effective service delivery.
• Discharge To Assess Service focuses on enabling adults to transition smoothly from hospital to community, preventing readmissions and fostering a safe discharge process. Work closely with healthcare partners to assess adults post-discharge to prioritise recovery and independence in a supportive environment.
