Job Description:

Role Summary

  • The post holder will work across a range of specialty Acute Medicine functions, primarily in Acute Hospital at Home (AHAH) and Outpatient Parenteral Antimicrobial Therapy (OPAT), including Triage and Same Day Emergency Care, as well as AHAH northern services.
  • They will lead in delivering seamless transition between the acute trust and the community, ensuring patient safety and high-quality care. Current patient streams include OPAT, AF, COVID, AKI, Heart Failure, NSTEMI, Syncope, Palliative, and Frailty, with new streams to be introduced.
  • They will proactively recruit new patients from AMU, SDEC, and wards, assess them, and facilitate their safe discharge into AHAH.
  • Their role includes improving and maintaining the AHAH service based on best practices and national guidelines.
  • They will collaborate closely with the AMU nursing team to support triage nurses, including patient streaming within SDEC, demonstrating good triage practice.
  • Ongoing learning and advanced practice modules, including history taking and clinical examination, are expected.

We are offering a unique opportunity for a Registered Nurse to join our innovative Virtual Ward team, bringing experience to a multidisciplinary team that includes Triage, Cardiac assessment, Outpatient Parenteral Antibiotic Therapies, Home, and Ambulatory Care.

The Virtual Ward provides a safe, efficient alternative to hospital beds, supporting patients in their homes to prevent admissions or enable early discharge. The role involves remote patient care using devices feeding into Epic, telephone consultations, face-to-face reviews, and ward visits across the Royal Devon to support patient flow.

This position supports the national priority of digitally enabled care, embedding technology to improve outcomes and reduce hospital pressure. The role also involves quality improvement and working with multidisciplinary teams.
