A leading community health organization is looking for a Clinical Nurse Specialist (CNS) to join their dedicated team in Exeter. This role focuses on delivering high-quality, evidence-based palliative care to patients with life-limiting illnesses. You will work collaboratively with other healthcare professionals, ensuring that patient dignity and quality of life are prioritized. This community-based role involves travel across Exeter and surrounding areas and participating in a seven-day service provision.

As a registered nurse with experience in palliative care or a related field, you will work with our multi-disciplinary team to provide exceptional, evidence-based, person-centred care for patients with complex, life-limiting illnesses. You will manage a patient caseload, offer expert advice and interventions, and collaborate effectively with local GPs and Community Nurses as part of Hospiscare’s renowned specialist care.

Responsible For: N/A
Accountable to: Head of Community and Supportive Care Services
To deliver an excellent standard of evidence-based specialist palliative nursing care and clinical advice.
To promote dignity, encompassing a person-centred approach, to positively enhance the lives of patients in the community with complex, life-limiting illnesses.
Maintain a safe environment for patients, visitors, and staff by adhering to infection prevention policies and reporting risks.
This role requires an enhanced DBS check with Adults and Children Barred Lists.
