Job description

This will be a permanent role within the Acute Diabetes Team for an experienced Diabetes Specialist Nurse to assess, develop and implement programs of care providing specialised advice on specialist diabetes care.

Practise in compliance with your respective code of professional conduct and within your scope of practice. Being responsible and accountable for decisions, actions, and omissions at this level of practice.

The post holder will work in line with The TREND Integrated Care and Competency Framework for Adult Diabetes Nursing to Senior Practitioner level.

Liaise with non-specialist teams in the development of management plans for patients with diabetes related problems including recurring hyperglycaemia, hypoglycaemia fear or unawareness, renal disease, partial sight, diabetic foot disease, preconception planning and pregnancy.

Provide the diabetes service within outpatient and inpatient settings within speciality areas.

Main duties of the job

*Support strategic development and operational delivery of care.

*Works independently and as part of MDT within inpatient and outpatient settings.
