Mersey West Lancashire NHS Trust is hiring an Acute Frailty Unit Navigator to join the team at Southport/Formby area. You will collaborate with registered nurses and other professionals to deliver high-quality, patient-centred care across ambulatory and ward settings.

You will contribute to ongoing assessments, support safe discharges, and maintain comprehensive clinical records in line with service pathways and departmental policies.

Qualifications

• Diploma or NVQ3 Health & Social Care qualification
• Courses relating to diploma/NVQ3 Health & Social Care
• Experience in health & social care settings
• Knowledge of safeguarding procedures
• Ability to work in a multi-disciplinary team
• Venepuncture, Cannulation, and ECG training

Responsabilités

• Deliver high quality, person-centred care under the direction of a Registered Nurse or other registered care professional.
• Work within NHS Values and as part of a designated clinical team.
• Assist with care planning, monitoring, and discharge procedures in ambulatory and ward settings.
• Collaborate with the multi-disciplinary team to support patients and families.
• Maintain high standards of clinical documentation and compliance with policies.
