Job summary

Mersey and West Lancashire Teaching Hospitals NHS Trust is seeking an Acute Frailty Unit Navigator to work with a multi-professional team, providing high quality, person-centred care under the supervision of a Registered Nurse. The post involves ambulatory and ward duties, contributing to discharge planning and care continuity in a Trust open Mon-Fri with a view to 7 day working.

You will hold a Diploma or equivalent Health & Social Care qualification, have experience in a multi-disciplinary

Qualifications

• Diploma or equivalent in Health and Social Care or similar.
• Diploma/NVQ 3 related courses completed.
• Knowledge of safeguarding vulnerable adults procedures.
• Ability to work in a multi-disciplinary team with supervision.

Responsibilities

• Deliver high quality, person-centred care under RN direction.
• Work with NHS values and team to meet patient needs.
• Contribute to care that focuses on individual requirements.
• Carry out delegated tasks under supervision to a high standard.
• Work autonomously in clinic and ward within defined plans.
