Mersey and West Lancashire Teaching Hospitals NHS Trust seeks an Advanced Clinical Pharmacist – Frailty to join the Frailty Pharmacy team. Based at Whiston Hospital, the postholder will deliver a clinical pharmacy service to frailty patients and work with multi-disciplinary teams to ensure quality, cost-effective pharmaceutical care.

Fixed term, 37.5 hours per week, 12 months maternity cover. Experience with acutely unwell patients and non-medical prescribing (or working towards) is essential;

Qualifications

• Experience providing pharmaceutical care to acutely unwell patients.
• Non medical prescribing qualification or working towards it.
• Strong teamwork with excellent communication and attention to detail.

Responsabilités

• Provide advanced clinical pharmacy services to frailty wards.
• Collaborate with pharmacy, medical and nursing teams to optimise medicines.
• Develop and deliver advanced pharmacist roles within the frailty service.
