Job summary

Berkshire Healthcare NHS Foundation Trust in Reading is recruiting an Activity Co-Ordinator for Rowan Ward to plan, coordinate and deliver person-centred activities for older adults with dementia and mental health conditions.

You will work closely with patients and families, lead a range of therapeutic activities, assess wellbeing, document sessions, and report progress to the multidisciplinary team. The role may include weekend shifts and rotating pattern, with 5 days a week initially.

Qualifications

• Experience planning therapeutic activities for in-patients.
• Ability to assess well-being during activities.
• Ability to report progress to the MDT.
• Prepare equipment and manage ward activities.
• Ability to adapt activities for dementia and older adults.
• Commitment to safety and risk assessment.

Responsibilities

• Lead planning of a range of activities appropriate for patients with dementia and mental health needs.
• Establish therapeutic engagement and relationships with inpatients and carers.
• Assess patient wellbeing during activities and monitor progress.
• Adapt activities to meet individual needs and maintain a calming approach.
• Provide verbal and written feedback to the multidisciplinary team after activities.
• Document each session in the patient care record.
• Risk assess all activities and review risk assessments for participants.
• Agree on equipment with Ward Manager and maintain activity equipment.
• Engage in violence prevention training, including physical intervention when required.
