Job summary

NHS Lanarkshire seeks a Pharmacist to provide patient-centred pharmaceutical care in secondary care and to support prescribing in primary care as needed.

You will work with consultants and multidisciplinary teams, delivering education to patients and reviewing medicines to ensure safe and economical use of medicines across acute medicine, surgery and emergency care.

Qualifications

  • Must have MPharm and be registered with GPhC.
  • Independent prescribing qualification or undertakes to complete within reasonable timescale.
  • Ability to work under pressure.
  • Ability to plan, prioritise and organise workload.
  • Experience in patient information/education.
  • Ability to effectively monitor patients’ therapeutic regimens and adjust therapy as needed.
  • Able to work under own initiative and as part of a team.
  • Commitment to revalidation in line with GPhC.
  • Good verbal and written communication skills.

Responsibilities

The Role

To provide patient focused quality pharmaceutical care and to assure an efficient and timely supply of pharmaceutical advice and medication within secondary care setting. To assist in the delivery of pharmacotherapy and prescribing support within a Locality in primary care setting. The NHS Lanarkshire Pharmacy Service aims to provide the highest quality pharmaceutical care to the people of Lanarkshire. The pharmacy team provide person-focussed pharmaceutical care to individuals, and supply medicines through systems that ensure safe, effective and economical use. We strive to ensure patients derive maximum benefit and minimum harm from their medicines, throughout their healthcare journey. We work in partnership with our clinical colleagues, providing high quality care, timely information and advice to deliver the safe and secure use of medicines. By working closely with colleagues across NHS and Health and Social Care Partnership (HSCP) services, we ensure medicines are purchased, stored, dispensed and prescribed to the highest standards in every care setting.

In This Key Role, You Will
  • To develop competence and to deliver patient-centred pharmaceutical care to patients in various specialities including acute medicine, acute surgery, emergency care and / or primary care.
  • To participate in consultant ward rounds and multidisciplinary team meetings as directed by senior pharmacists.
  • To deliver patient education to ensure the safe and effective use of medications and devices
  • To deliver clinical care to patients by assessment of their pharmaceutical care needs and developing and actioning a care plan.
  • To review and analyse both medicine and patient information and provide specialist advice on drug administration and changes to therapy accounting for individual patient needs and clinical condition and local management protocols
What You'll Bring
  • Masters of Pharmacy Degree
  • Registered member of General Pharmaceutical Council
  • Independent Prescribing qualification or undertakes to complete within reasonable timescale.
  • Ability to work under pressure.
  • Able to plan, prioritise and organise workload
  • Experience in patient information/education
  • Ability to effectively monitor patients' therapeutic regimens in order to ensure effectiveness and reduce toxicity
  • Able to work under their own initiative and as part of a team.
