Job summary

NHS Lanarkshire is seeking a pharmacist to deliver patient-centred pharmaceutical care across acute and primary care settings. The role involves participation in consultant ward rounds and MDT meetings, and providing education to ensure safe use of medications and devices.

You will assess pharmaceutical care needs, develop care plans, and review medicines to advise on therapy changes, working closely with clinical colleagues to maintain high standards of care.

Qualifications

  • Must hold a Master of Pharmacy degree.
  • Registered with the General Pharmaceutical Council (GPhC).
  • Independent Prescribing qualification or willing to obtain within a reasonable timeframe.
  • Ability to work under pressure.
  • Able to plan, prioritise and organise workload.
  • Experience in patient information/education.
  • Ability to monitor patients’ therapeutic regimens to ensure effectiveness and minimise toxicity.
  • Able to work independently and as part of a team.
  • Commitment to revalidation as per GPhC requirements.
  • Strong verbal and written communication skills.

Responsibilities

  • Deliver patient-centred pharmaceutical care across acute and primary care settings.
  • Participate in consultant ward rounds and multidisciplinary team meetings.
  • Provide patient education to promote safe and effective use of medications and devices.
  • Assess pharmaceutical care needs and develop and action care plans.
  • Review medicines and patient information, offering specialist advice on drug administration and therapy changes.
