Job summary

The Role

This post represents an exciting opportunity to join a dynamic, highly skilled specialist rehabilitation team. The South Stroke neuro Team is based at Udston Hospital and treats both stroke and neurological patients. The post holder should ideally have experience of stroke and neurology at band 3 or 4 level.

To work under the supervision of registered therapy staff to assist patients to achieve their rehabilitation goals.

Undertake baseline assessments within clearly defined parameters for the identified service.

Implement, adapt and progress treatment programmes within agreed protocols and defined parameters.

Use specialist technical and practical skills to deliver flexible, patient centred programmes across a variety of clinical settings.

Discharge or transition care within agreed protocols and defined parameters

In this key role, you will:

Responsible for a delegated clinical caseload of patients whose treatment follows clearly defined parameters.

Assess, implement, progress treatment and discharge in accordance with agreed protocols and parameters.

Plan and prioritise delegated patient caseload.

Plan and organise a range of therapeutic activities/clinics

Deliver prescribed education sessions including patient/carer advice to selected groups of patients.

Provide updates to the registered therapist regarding patients’ progress and response to treatment.

Maintain effective communications with patients, relatives, carers, registered therapy staff, members of the multi-disciplinary team and other agencies to ensure appropriate information is shared and patient needs are met.

What You'll Bring:

HNC in healthcare, education or social cares or SVQ III in healthcare, education or social care or equivalent experience in a health, education or social care environment.

Short specialist courses relevant to specific clinical area.

Full driving licence holder.

Awareness of health legislation relevant to the clinical area.

Knowledge of the therapeutic process, techniques and / or activities relevant to client group.

Basic assessment skills.

Ability to apply a range of technical and practical rehabilitation skills.

Effective listening, communication and interpersonal skills.

Time management, planning and organisational skills.

Working knowledge of basic information technology.

Ability to work individually, or as part of a team

Ability to learn and develop new skills and maintain a record of learning.
