This is an exciting opportunity to join an established Community Nursing workforce and be part of the Stewartry Home team.
This Band 5 post covers Gardenhill, Dalbeattie, and Kirkcudbright GP surgeries in the heart of Stewartry. You must be NMC registered, and a driving licence is essential. If a disability prevents you from holding a driving licence, consideration will be given if alternative transport can be arranged.
You will be expected to support patients registered with all three practices. The hours are full-time; however, part-time hours may be considered.
The post holder will provide high-quality, effective care within the community and work in partnership with patients, healthcare professionals, care providers, and other agencies. Clinical responsibilities include wound management, catheter care, and supporting patients at end of life.
The service aims to focus on prevention, early intervention, and supporting self-management to enable people to live longer, healthier lives at home. Good communication skills and the ability to work autonomously within the team are essential.
The Community Nursing Service operates an open access policy, available 365 days a year, 24 hours per day. The service does not operate on a waiting list; care needs are prioritized, and the team responds to planned and unplanned care. Unscheduled visits and urgent interventions should be responded to promptly. The post holder will deliver care delegated by the Community Charge Nurse. The caseload population is derived from the GP practices aligned to the service.
