Job summary

The Terraces Orleans in Orleans, Massachusetts is seeking an experienced Weekend Nurse Supervisor to lead weekend nursing operations, oversee resident care, staffing, documentation, and safety. You will serve as the clinical lead on weekend shifts, with support from DON, ADON, and Administrator.

Ideal candidates are RN or LPN with leadership experience, proficient in PCC, and committed to high care and documentation standards during off-site management hours.

Qualifications

• Current Massachusetts RN or LPN license and CPR certification.
• Two years of long-term care or skilled nursing experience preferred.
• Strong clinical judgment, leadership, communication, and organizational skills.
• Working knowledge of PointClickCare (PCC) and long-term care regulatory requirements.
• Professionalism, accountability, clear communication, and timely follow-through.

Responsibilities

• Supervise nursing staff and CNAs; round on units to assess resident care, staff performance, and safety.
• Provide coaching and corrective feedback on incomplete assignments, documentation, treatments, and care concerns.
• Respond to call-outs, secure coverage, adjust assignments, and communicate significant staffing concerns to nursing management.
• Review PCC records for accurate, timely nursing notes, change-in-condition assessments, incidents, and admission documentation.
• Verify CNA point-of-care documentation for ADLs, showers, bowel movements, vital signs, and weights.
• Follow up on missing entries and outstanding tasks before shift end whenever possible.
• Verify new physician orders are entered, acknowledged, implemented, and communicated; hand off unresolved items to the oncoming shift and ADON/DON.
• Assess changes in condition and ensure appropriate physician/NP and family notification and follow-up.
• Coordinate admissions, transfers, discharges, and hospital returns; monitor medications, treatments, infection control, and safety.
• Investigate incidents and falls, document findings, and ensure immediate interventions.
• Collect nonurgent requests from units and coordinate routine on-call communication when clinically appropriate.
• Escalate urgent concerns immediately; ensure new orders and recommendations are implemented and documented.
• Act as weekend clinical lead; respond to emergencies and resident/family concerns.
• Support completion of in-service and Relias education for weekend staff.
• Notify the DON and Administrator immediately of any significant incidents, abuse allegations, or staffing or regulatory concerns; coordinate with support departments.
• Confirm nursing and CNA assignments and PCC follow-up; provide a thorough handoff.
• Complete a weekend supervisor report covering staffing, incidents, admissions, clinical follow-up, and unresolved matters for DON review.

The Terraces Skilled Nursing is seeking an experienced, confident nurse leader to serve as our Weekend Nurse Supervisor. In this role, you will be the clinical lead for the building on weekend shifts, overseeing nursing operations, resident care, staffing, clinical documentation, and facility safety.

Responsibilities

Nursing Supervision & Staff Accountability
