This travel assignment offers 8-hour day shifts, 40 hours per week, and a 13-week duration. Licensure in NC and CPR/BLS certification are required for timely credentialing.

Qualifications

• PT license in good standing and NC licensure or eligibility.
• BLS certification required.
• Experience in Skilled Nursing Facility or post-acute rehab preferred.

Responsibilities

• Perform physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on patient needs and functional goals.
• Provide therapeutic exercise, gait training, balance training, transfer training, and mobility interventions.
• Help patients improve strength, endurance, safety, and independence.
• Educate patients and caregivers on mobility strategies and home exercise programs.
• Collaborate with nursing staff, OT, SLP, and the interdisciplinary rehab team.
• Complete accurate and timely documentation.
