Theoria Medical is seeking an Advanced Practice Provider to deliver high‑quality care to residents in skilled nursing facilities. You will round daily, formulate treatment plans, and address acute issues while collaborating with a clinical team.

Qualifications

• Active U.S. license and board certification in ANCC, AANPCB, PNCB, AACN (Nurse Practitioners) or NCCPA (Physician Assistants).
• Experience in skilled nursing facilities preferred.
• Ability to work independently with flexible scheduling.

What You’ll Do

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
