Jefferson County Human Services in Wisconsin is seeking a Children’s Long-Term Support (CLTS) Services Coordinator to deliver assessment, case planning, protection, and case management for children with developmental, emotional, or physical disabilities.

This full-time position requires a Bachelor’s degree in social work or a related field with one year of experience (three years preferred), plus a valid driver’s license and CLTS waiver certification within 30 days of hire.

Qualifications

• Bachelor’s degree in social work or related field with at least one year of experience.
• Equivalent combination of education and experience may be considered.
• CLTS Waiver Certification must be obtained within 30 days of hire.
• Valid Driver’s License is required.

Responsabilités

• Provides assessment, case planning, protection, and case management services in compliance with Federal and State regulations.
• Develops and reviews individual service plans through direct assessment of health/physical functioning, AODA, mental health, and financial well-being; utilizes available resources.
• Determines if a family or consumer is in crisis, assesses safety and risk, and coordinates with other programs to address the crisis.
• Refers clients to community resources and facilitates ongoing support services.
• Connects families to support throughout the community and promotes self-determination and involvement in community life.
