Job summary

Choices CCS is seeking a Wrap Facilitator - Family Preservation for a hybrid role in Versailles, IN. You will perform comprehensive strengths-based assessments, partner with the team to develop a care plan, oversee its implementation, and connect families with services and resources.

You will conduct regular home visits, monitor services, and ensure data is collected for evaluation while maintaining fiscal accountability and cultural competence.

Qualifications

• Bachelor's degree in social work or related human service field.
• Two years experience partnering with youth and/or families within the context of social service or education.
• Ability to obtain CANS certification within 45 days of hire and maintain thereafter.
• Strong communication and writing skills; bilingual Spanish a plus.

Responsibilities

• Complete strengths-based assessments for individuals and collaborate with the team to develop a care plan.
• Oversee implementation of the plan and coordinate services with providers and family resources.
• Facilitate monthly community team meetings, conduct home visits, and monitor all services for client care.
• Gather and record data for program evaluation and ensure fiscal accountability.
